Making an appointment?

Here are a few tips to make your exam experience a breeze.

New to our office? please arrive 10-15 minutes early to complete New Patient forms, to allow us to find your insurance policy in our system, and make sure you'll be seen at your scheduled appointment time.

 

To save our patients time in the waiting room, we offer a printable New Patient form for your convenience. Feel free to fill it out at home before your appointment!

Pediatric Eye Exams, Hollaway Eye Associates

Our office does not perform pediatric eye examinations. Our facilities are not specifically equipped to perform examinations on small children, and most of our doctors do not specialize in pediatric care. 

 

If you are unsure whether your child meets the criteria for pediatric care, please call our office to speak to one of our technicians. We'll be more than happy to recommend alternative pediatric optometrists and ophthalmologists in your area.

If you are visiting our office for an DoDMERB/ROTC examination, please do not wear contact lenses for 72 hours prior to your appointment.

 

Be sure to speak with your recruiter beforehand to assure that all required paperwork has been sent to our office prior to your scheduled appointment time.

Vehicle Tint Waivers, Hollaway Eye Associates

Our office does not sign vehicle tint waivers under any circumstances. A medical tint waiver allows a vehicle to have legally tinted windows for an existing medical condition. You may need to contact your primary care physician or a specialist in order to have a tint waiver signed.
